1. Tax Payer SSN
2. Date of Birth
3. Refund Amount
4. Filing Status
   

   

Resetting Authorization

If a Republic Bank check has been authorized but not cashed, the check would need to have the authorization reset so it can be authorized again.

To reset the authorization of a Republic Bank check, refer to the following steps below: 

1. Obtain an Authorization Reset Form from Republic 
   888-676-2056
   https://www.republicbank.com/
   
2. Complete the form and secure the original check.
3. Send the completed form and original check by email to EPForms@republicbank.com or by fax at 1-866-300-3528.
