Symptoms

’Package Not Loaded for Form : 01’

   

Cause

Ghost data appears to be present

Resolution

1. From the Return Menu
2. Select Delete State
3. Click on the "dead" space within the Delete State window
4. Select OK 
5. Select OK in the Delete State Package window that populates
6. Verify that the error is cleared
